A home care assessment aims to comprehensively evaluate an individual’s needs and surroundings to establish the most suitable care plan.
After the assessment, our team collaborates with the individual and their family to determine the appropriate care type: skilled nursing, personal care, companion care, or respite care.
Expect the following areas to be covered in a standard in-home assessment:
Background
Understand why you seek at-home care, such as accidents, surgeries, or aging-related decisions.
Demographics
Identify factors influencing physical interactions and communication with the individual and other caregivers.
Activities of Daily Living (ADLs)
Determine if the person can perform daily tasks independently or requires assistance, like dressing, bathing, eating, etc.
Instrumental Activities of Daily Living (IADLs)
Assess assistance needs for medication management, shopping, traveling, and telephone use.
Medical Conditions & Allergies
Evaluate chronic or acute health conditions, recent medical stays, hearing, speech, vision, allergies, etc.
Mental/Behavior Conditions
Explore memory loss, anxiety, repetition, sleeping problems, and other behavioral conditions to optimize the living environment.
Exercise
Consider any specific exercise routines or rehabilitation needs the individual might have.
Equipment/Environment
Inspect safety features like bed rails, shower seats, and other assistive equipment to ensure a secure living space.
Pet Care
Addresses the care requirements of pets that provide companionship to the individual.
